We have a few forms that we require our clients to fill out and sign prior to their first appointment.  Save some time at your appointment by downloading the following forms (using the down arrow icon in the upper right corner of each form), filling them out at home and either bringing them to your appointment or emailing the completed and signed copies to us at courtney@redgept.com.
